What is the DAISY Award?

The DAISY Award For Extraordinary Nurses is an international recognition program that honors and celebrates the skillful, compassionate care nurses provide every day.
 
DAISY is an acronym for Diseases Attacking the Immune SYstem. The DAISY Foundation formed in 1999 by the family of Patrick Barnes as a way to honor the nurses that provided quality and compassionate care to Patrick during the final stages of his battle with idiopathic thrombocytopenic purpura (ITP). Today, there are over 2,600 healthcare facilities in all 50 states and 15 other countries committed to honoring nurses with The DAISY Award. Lone Peak Hospital is proud to be a DAISY Award Partner.

What is the Moving Mountains Award?

The Moving Mountains Award was established to recognize patient care partners (all non-nursing colleagues) who demonstrate that kindness, compassion and talent moves mountains for our patients.


Nominee information

Nominate an extraordinary employee of Lone Peak Hospital as a deserving recipient of The DAISY Award or the Moving Mountains Award if they consistently meet all of the following criteria:

  • Goes above and beyond to care for the needs of the patient and family
  • Has a positive attitude and demonstrates professionalism
  • Job performance exemplifies the mission, vision and values of Lone Peak Hospital
  • Is thoughtful, compassionate and caring in all situations
  • Is energetic and passionate about patient care
