Observer required documentation
St. Mark's Hospital has a long history of participation in education, and we would like to honor and continue that tradition. We are appreciative of the opportunity to provide you with an observation experience. The following requirements & documentation must be completed, compiled & turned in to the Medical Staff Services Office. Your documentation will then be reviewed. Once all requirements have been met, you will receive an email clearing you to begin your observation.

- Observer Profile Form.
- Observer Orientation Booklet and complete the post-test on the Answer Sheet provided. *Not Required for HCA employees*
- Statement of Responsibility
- Confidentiality and Security Agreement
- Parking Policy Agreement
- The requirement to provide a copy of their driver’s license. *Not Required for HCA employees*
- The requirement to provide a copy of the Healthcare Professional license.
- The requirement to provide a copy of TB documentation (details on Observer Profile Form). *Not Required for HCA employees*
- The requirement to provide documentation of a current/annual Influenza Vaccination *For observation dates between Oct 1-Mar 31 or as determined by CDC*
The observer must obtain their own sponsor, a St. Mark’s Hospital employee or physician, who is willing to allow observation of their clinical practice. St. Mark’s will not make these arrangements on the observer’s behalf.
